Hoosier Healthwise
Approximately 270,000 members

This plan provides health coverage for children, pregnant women and low-income families. The Children’s Health Insurance Program (CHIP) is part of Hoosier Healthwise. The level of coverage available to members depends on which Hoosier Healthwise benefit package they qualify for. Eligibility is based on the number of family members and household income. Indiana Care Select

MDwise is a Care Management Organization for the Indiana Care Select Program. The Indiana Care Select program is designed by the State of Indiana to provide disease and care management services to the aged, blind, physically and mentally disabled, wards of the court and foster children, and children on adoptive services. The disease management programs that MDwise provides our members in Care Select include: Asthma, Diabetes, Congestive Heart Failure, Cornonary Artery Disease, Hypertension, Chronic Kidney Disease, Severe Mental Illness, Serious Emotional Disturbance and Depression. |

Healthy Indiana Plan (HIP)

MDwise provides a full scope of health coverage to low income, uninsured Hoosiers, ages 19-64. The Healthy Indiana Plan is designed by the State of Indiana to offer a basic health insurance benefit plan, subject to individual contributions, and paired with a personal health account referred to as a Power Account to Hoosiers without access to employer sponsored health insurance. The Program is designed to foster personal responsibility and promote preventive care and healthy lifestyles while encouraging participants to be value conscious consumers of health care. |
